Issued 2000 by the American Lung Association. Designed by Elaine Verstraete. Watercolor Snow Scenes: Brick House, Sleigh, Church, Covered Bridge. Gift tags in 6th column along the right side of the sheet. Gift Tags have two different designs; cardinal and wreath pn a door. All within red border. Sheet is SE 4S, wide margin at top.

Gummed rouletted sheet, yellowish rubbery gum, white simulated perfs. with "Financial Information Available ...." on reverse of sheet. Printer- Moore Response Marketing Services.
